As the COVID-19 pandemic has demonstrated, diseases know no borders. Globally, we are more interconnected and mobile than ever before, making a health threat anywhere a health threat everywhere.
To safeguard and promote the health of its citizens, every country must have effective field epidemiology capacity.
Field epi capacity means we can detect, investigate and control health threats; carry out health surveys; conduct disease surveillance; establish/evaluate surveillance systems; perform applied field research, and evaluate prevention/control efforts.
Field epidemiology (AKA applied, interventional, or “shoe leather” epidemiology) is conducted with the aim of taking action to address a public health problem—linked to human, animal or environmental health—to reduce illness, injury or death.
Field epidemiology links data to action to design or improve policies and interventions to protect population health, especially in response to unexpected health problems when rapid on-site investigation is necessary.
The traditional concept of field epidemiology and the roles of field epidemiologists are expanding. More than ever, field epidemiologists are directly involved in developing innovative solutions to public health problems.
The International Health Regulations (IHR) and the Global Health Security Agenda (GHSA) call for at least 1 trained field epidemiologist per 200,000 people in every country to detect, respond to, and contain public health emergencies.
Field epidemiology is key to strengthening epidemiologic and surveillance capacity at all levels of the health system, mitigating public health threats, and leading to reduced mortality and case numbers, epidemic duration, and potential for spread.

Key Messaging on Field Epidemiology Training Programs (FETPs)

As countries’ health systems face increasingly complex threats, training workers in field epidemiology is even more important. The globally recognized Field Epidemiology Training Program (FETP) builds a cadre of skilled field epidemiologists.
FETP builds capacity within health service agencies by training the public health workforce in field epidemiology and other competencies in the context of health delivery systems.
More than 18,000 public health workers have trained through FETP. Currently, there are more than 80 programs serving 160+ countries and territories.
FETP provides countries with critically needed public health services through a mentored, learning-by-doing approach that emphasizes fieldwork and improves the effectiveness of the workforce and the systems required to provide public health services.
FETP is essential to global health security as it enables countries to respond more quickly and effectively to threats within their borders.
FETP trainees and alumni have provided pivotal support to health emergencies including COVID19, Ebola, Zika, yellow fever, and natural disasters such as Cyclone Idai in Mozambique.
FETP contributes to the strengthening of the public health system as a whole. The majority of alumni stay within the public health system, and many take on positions of leadership, forming a culture of using data for decision-making.
Collectively, FETP trainees have conducted more than 9,350 outbreak investigations and established or evaluated more than 3,800 disease surveillance systems since the FETP initiative began in 1975.
Field Epidemiology and Laboratory Training Programs (FELTPs) and Field Epidemiology Training Programs for Veterinarians (FETPVs) incorporate laboratory and veterinary components in their core training curricula.
Developing a highly qualified animal health workforce, via FETP-V and ISAVET and other means, is vital for ensuring that countries have the human resources available to detect and respond to emerging zoonoses and other health security risks.
